要約 目的:後期高齢者の緑内障診断での網膜神経線維層厚解析の評価。症例と方法:緑内障が疑われた273名544眼にHumphrey視野検査と網膜神経線維層厚解析(GDxVCC)を行った。40~59歳の中高年齢者380眼と,75~100歳の後期高齢者164眼の結果を比較した。結果:信頼できる視野は,中高年齢者の75.3%,後期高齢者の46.3%で得られ,有意差があった(p<0.001)。信頼できるGDxVCC結果は,それぞれ90.3%と65.9%で得られ,有意差があった(p<0.001)。緑内障が確定した80眼では,視野の平均偏差とGDxVCCの神経線維指数との間に相関があった(中高年齢者r=-0.65,後期高齢者r=-0.68,p<0.001)。結論:GDxVCCによる網膜神経線維層厚解析は,結果の信頼性が高く,視野と相関し,後期高齢者の緑内障診断に有用である。
Abstract. Purpose:To report the value of retinal nerve fiber thickness analyzer(GDxVCC)in the diagnosis of glaucoma in senile persons. Cases and Method:This study was made on 544 eyes of 273 persons who were suspected of glaucoma. The series comprised of 2 groups:middle-aged persons(40 to 59 years,380 eyes)and senile persons(75 to 100 years,164 eyes). All were tested by Humphrey perimetry and GDxVCC. Results:Reliable perimetric data were obtained in 75.3% of middle-aged and 46.3% of senile persons. The difference was significant(p<0.001). Reliable GDxVCC data were obtained in 90.3% and 65.9% respectively. The difference was significant(p<0.001). In 80 eyes with definite glaucoma,there was a positive correlation between mean deviation of visual field and nerve fiber parameter by GDxVCC. Conclusion:GDxVCC is useful in the diagnosis of glaucoma in senile persons due to high rate of reliable data.
