黄斑上膜の術前後変視変化の定量，早期手術は有用か

Value of early surgery and quantitation of metamorphopsia before and after surgery

要約　目的：変視症を軽減する目的での黄斑上膜の手術時期の検討。対象と方法：過去9年間に手術を行った特発性黄斑上膜94例99眼を対象とした。男性47眼，女性52眼で，年齢は平均69.3±7.4歳であった。術前と術後6か月以降にM-CHARTSの縦方向と横方向の平均値で変視症を評価した。結果：全99眼での変視症の平均値は，術前後で有意差はなかった。術前のM-CHARTS値が0のものは術後に有意に悪化し，0.05～0.5のものでは不変，0.55以上であれば有意に改善した。結論：黄斑上膜による変視症は膜除去により改善が期待できる。術前のM-CHARTS値が小さい初期の症例では再検討が必要である。

Abstract.　Purpose：To report the optimum timing of surgery for epiretinal membrane regarding improvement of metamorphopsia. Cases and Method：This retrospective study was made on 99 eyes of 94 cases of idiopathic epiretinal membrane who received surgery in the past 9 years. The series comprised 47 males and 52 females. The age averaged 69.4±7.4 years. Metamorphopsia was quantitated using M-CHARTS before and 6 months after surgery. Results：There was no change in averaged value of metamorphopsia before and after surgery. The value by M-CHARTS significantly deteriorated when it was zero before surgery,remained unchanged when it was between 0.05 and 0.5,and improved when it was 0.55 or over. Conclusion：Metamorphopsia improves after surgical removal of epiretinal membrane. Epiretinal membrane in its early stage may be an exception.
