強膜トンネル併用円蓋部基底結膜切開線維柱帯切除術の手術成績

Outcome of trabeculectomy with scleral tunnel and fornix-based conjunctival flap for open-angle glaucoma

要約　目的：強膜トンネル併用円蓋部基底結膜切開線維柱帯切除術（本手術と略称）の結果の報告。対象と方法：本手術を行った17眼を対象とした。3眼に正常眼圧緑内障，14眼に原発開放隅角緑内障があり，術前眼圧は平均23.3mmHgであった。結果：平均眼圧は術後12か月で10.7mmHg，24か月で12.0mmHgと，有意に低下した（p＜0.005）。目標眼圧を20mmHg以下，15mmHg以下，術前眼圧の30％以上低下とした場合の最終眼圧コントロール率は，それぞれ94％，82％，100％であった。結論：正常眼圧緑内障または原発開放隅角緑内障に対し，本手術で有意な眼圧下降が得られる。

Abstract.　Purpose：To report the outcome of trabeculectomy with scleral tunnel and fornix-based conjunctival flap for glaucoma. Cases and Method：This retrospective study was made on 17 eyes that received this surgery. The series comprised 3 eyes of normal-tension glaucoma and 14 eyes of primary open-angle glaucoma. Intraocular pressure（IOP）before surgery averaged 23.3 mmHg. Results：IOP averaged 10.7mmHg 12 months and 12.0 mmHg 24 months after surgery. Both decreases in IOP were significant（p＜0.005）. Final IOP decreased below 20 mmHg in 94％ and below 15 mmHg in 82％ of eyes. It decreased by 30％ below the initial value in 100％ of eyes. Conclusion：The present surgical modality induces significant decrease of IOP in eyes with normal-tension or primary open-angle glaucoma.
