涙囊に発生した化膿性肉芽腫の1例

A case of pyogenic granuloma of the lacrimal sac

要約　目的：涙囊部に生じた化膿性肉芽腫の症例の報告。症例：86歳女性が左涙囊部の腫脹で受診した。2年前から涙囊炎として治療を受けていたが，最近になり腫脹が増大した。所見：左内眼角の下方に20mm×15mmの発赤と腫脹があり，圧痛は軽度であった。涙管の通水はなく，膿や血の逆流はなかった。CTで涙囊部に15mm×8mmの均一な陰影があった。涙囊を切開したところ内腔に腫瘤塊があり，涙囊を摘出した。病理学的に，毛細血管の増生と炎症細胞の浸潤がある肉芽組織であり，悪性ではなく，化膿性肉芽腫と診断した。結論：化膿性肉芽腫が涙囊炎に類似する所見を呈することがある。

Abstract.　Purpose：To report a case of pyogenic granuloma of the lacrimal sac. Case：A 86-year-old female presented with swelling in the left lacrimal sac region. She had been treated under the diagnosis of dacryocystitis with no improvement. Findings：Hyperemia and swelling was present in the left lacrimal sac area. Its size was 20 mm×15 mm. The lacrimal drainage system was obstructed. There was no backflow of blood or pus by compression. Computed tomography（CT）showed a homogenous lesion in the lacrimal sac area. Its size was 15 mm×8 mm. A tumor mass was found in the lacrimal sac after incision. The removed tissue showed proliferation of capillaries, infiltration of inflammatory cells, and no malignancy. The finding was diagnosed with pyogenic granuloma. Conclusion：This case illustrates that pyogenic granuloma may simulate chronic dacryocystitis.
