涙小管に発生した化膿性肉芽腫

Pyogenic granuloma of lacrimal canaliculus

　涙小管より発生し，涙点より突出した化膿性肉芽腫を経験した。症例は56歳の女性で，涙小管部近くに霰粒腫が合併していた。眼脂から菌は検出されず，涙道造影では狭窄はなかった。涙点より引き出して切除した腫瘤は，組織学的には，毛細血管の増生と炎症性細胞の混在する肉芽組織であった。腫瘤の根部は涙小管の屈曲部にあり，涙小管部近くの霰粒腫が涙小管に穿破し，化膿性肉芽腫が発生したと考えられた。この症例では，4回再発したが，涙小管内の腫瘤は全摘出が難しく，再発しやすいため，摘出方法の検討が重要であると思われた。

A 56-year- old female presented with pyogenic granuloma of lacrimal canaliculus protruding from the punctum in her left eye. A chalazion was pres-ent adjacent to the canaliculus. Bacterial culture of lacrimal discharge was negative. The lacrimal passage was patent. Excised specimen showed a granulation tissue containing capillary hypertrophywith inflammatory cells. The pyogenic granuloma had its root at the ampulla of the canaliculus and seemed to have grown after the chalazion ruptured into the canaliculus. The granuloma recurred 4 times after each attempted resection.
