Abstract. Purpose:To report a case of recurrent corneal epithelial defect that persisted after multiple penetrating keratoplasty(PKP)and that healed after treatment for herpes virus infection. Case:A 78-year-old male was referred for bullous keratopathy following cataract surgery 4 years before. His corrected visual acuity was counting fingers right and 1.0 left. His right eye showed bullous keratopathy with epithelial defect and corneal opacity. Clinical Course:PKP was followed by graft melting necessitating another PKP 4 months later. Due to similar events, a third PKP was performed another 2 months later followed by lamellar keratoplasty another 5 months later. Transplantation of amnion was a failure. Suspecting viral infection, instillation of acyclovir was started 23 months after his first visit. Fourth PKP was a success with transparent graft and no corneal epithelial defect. Conclusion:This case illustrates that viral infection may be present in recurrent corneal epithelial defect in spite of PKP.
