Abstract. Purpose:To report a case of severe vernal keratoconjunctivitis that improved by tacrolimus ointment. Case:A 13-year-old boy developed pain and impaired visual acuity in both eyes. He reportedly showed giant papilla in the palpebral conjunctivitis and corneal erosion and was diagnosed with vernal keratoconjunctivitis. He was treated by topical corticosteroid, antiallergic drug, and immunosuppressive agent for 10 months. He was referred to us for after treatments had failed. His corrected visual acuity was 0.5 in either eye. Injection of triamcinolone and resection of papilla proved futile. Two months later, he started receiving 0.1% tacrolimus ointment twice daily. Giant papilla in the conjunctiva and corneal lesions disappeared one month later with improved visual acuity to 1.5 in either eye. There has been no recurrence for 3 months after discontinuation of tacrolimus. There has been no adverse effect except burning sensation during the early phase of treatment. Conclusion:Topical tacrolimus may prove effective for severe vernal keratoconjunctivitis.
