要約 目的:早期治療を行った未熟児網膜症の報告。対象と方法:過去5年間に当施設で治療を行った未熟児網膜症75例148眼を対象とした。男児24例,女児51例であり,出生体重は392~1,410g(平均750g),在胎期間は22~29週であった。初回治療は未熟児網膜症が国際分類のstage 3 moderateの範囲が30°に達した時点でレーザー光凝固または冷凍凝固で行った。結果:初回治療は修正在胎期間31~41週(平均36.6週)で行われ,このときの未熟児網膜症はstage 3 moderateが60°に及ぶ例が最多であった。最終的に後水晶体線維増殖が2例4眼(2.9%)に発症した。結果:未熟児網膜症に対する早期治療は有効であり,zone 1網膜症の治療成績の向上が望まれる。
Abstract. Purpose:To report the results of early treatment for retinopathy of prematurity(ROP). Cases and Method:This study was made on 148 eyes of 75 newborn babies who were treated by us during the past 5 years. The series comprised 24 male and 51 female babies. The birthweight ranged from 392 to 1,410g, average 750g. The gestational age ranged from 22 to 29 weeks. Eyes received cryotherapy or laser photocoagulation when ROP reached stage 3 moderate over 30 degrees of circumference. Results:Initial treatment was performed at the corrected gestational age of 31 to 41 weeks, average 36.6 weeks. Stage 3 moderate of ROP had involved less than 60 degrees of circumference. Retrolental fibroplasia eventually developed in 4 eyes of 2 infants(2.9%). Conclusion:Early treatment is effective for ROP. Efforts are needed to improve the therapeutic outcome for zone 1 ROP.
