4歳以上で手術を行った両眼性先天白内障の視力予後

Visual outcome after surgery for bilateral congenital cataract in children aged four years or over

目的：小児期に手術を行った両眼性白内障の視力転帰の検討。対象と方法：4～10歳の間に両眼性白内障の手術を受け，2年以上の観察ができた12例24眼を対象とした。男児5例，女児7例であり，手術時の平均年齢は6歳7か月であった。術前に内斜視が2例あり，眼振はなかった。ERGまたは眼底に異常がある症例と発達遅滞などで視力の測定ができない症例は除外した。7例13眼に眼内レンズを挿入した。結果：全24眼で0.5以上，20眼で0.8以上の術後矯正視力が得られた。結論：眼振がない両眼性先天白内障では，4歳以後に手術を行っても良好な視力が得られた。

Purpose：To review the visual coutome after surgery for bilateral congenital cataract in children aged four years or over. Cases：This study involved 24 eyes of 12 patients who received surgery at the age of 4 to 10 years. The series comprised 5 boys and 7 girls. Their age averaged 6 years 7 months at the time of surgery. All cases were followed up for 2 years or longer. Esotropia was present in 2 cases. Nystagmus was absent. Cases were excluded who showed abnormal electroretinogram or funduscopic findings,or who could not be evaluated as to visual acuity due to mental retardation. Intraocular lens was inserted in 13 eyes of 7 cases. Results：Final visual acuity was 0.5 or over in all the 24 eyes and 0.8 or over in 20 eyes. Conclusion：Good visual acuity was obtained after surgery for bilateral congenital cataract in children aged four years or over without nystagmus.
