未熟児早期貧血治療と未熟児網膜症

Treatment of anemia and retinopathy of prematurity

（18-D501-34）　1992〜1996年の5年間に，当大学病院新生児集中治療室で管理した低出生体重児における未熟児網膜症の発症・進行と，未熟児早期貧血の治療との関連について検討した。出生体重2,500g未満の低出生体重児全体，1,000g以上1,500g未満の児，1,000g未満の児いずれにおいても，濃厚赤血球輸血を受けた輸血群33例は，非輸血群30例に対し有意に未熟児網膜症の発症・進行がみられた。この結果には児の未熟性に起因する輸血が必要な全身状態に加え，輸血による全身状態の急激な変化が関与していると考えられた。

We reviewed 63 premature babies during the foregoing 5 years regarding the relationship between anemia and retinopathy of prematurity (ROP) . All the cases weighed 2,500 g or less at birth. Incidence of ROP was significantly higher in 33 infants who received transfusion of adult blood than in 30 infants who did not. Progression of ROP was more severe in infants treated with blood transfusion. Above fact may be due to abrupt systemic changes by blood transfusion and to the underlying conditions necessitating blood transfusion.
