Abstract. Purpose:To report 2 eyes of posteriorly luxated lens fragment by bimanual phacoemulsification and aspiration(PEA)using an optical fiber-free intravitreal surgery system(OFFISS). Cases and Method:One was a 69-year-old woman with luxated lens following blunt ocular trauma. The other was a 59-year-old man with luxated lens fragments during routine cataract surgery. Both eyes were treated by PEA and OFFISS. Results:The current surgical modality enabled bimanual fixation of the lens or lens fragments. There was no intra-or postoperative complication. Both cases attained final visual acuity of 0.3 and 1.0 respectively. Conclusion:Luxated lens or lens fragments can be safely treated by bimanual PEA and OFFISS.
