Abstract. Purpose:To report a case of orbital apex syndrome caused by invasive aspergillosis which was brought under control by systemic voriconazol. Case:A 71-year-old diabetic woman presented with dull pain in the right orbit, impaired visual acuity and abnormal color sense of few weeks' duration. Findings:Her visual acuity was 0.07 right and 1.0 left. The right eye had abdens palsy. Diagnostic imaging was suggestive of fungal infection of paranasal sinus involving the orbital apex. Drainage of ethmoid and sphenoid sinus was followed by loss of vision in the right eye. After diagnosis of invasive aspergillosis, she started receiving paravenous voliconazol at the dosis of 500mg per day. The ocular motility became normal with visual acuity improving to finger counting. She has been doing well for one year until present. Conclusion:Systemic voriconazol may be effective for paranasal and orbital infection by invasive aspergillosis.
