眼サルコイドーシスにおいて網膜裂孔を生じた1例

Retinal tear in a case of ocular sarcoidosis

目的：網膜裂孔が生じた眼サルコイドーシス症例の報告。症例と所見：60歳女性が両眼の飛蚊症で受診した。矯正視力は右1.0，左0.7であり，両眼に虹彩毛様体炎，網膜滲出斑，網膜血管炎があった。鼻尖部皮疹の生検でサルコイドーシスと診断し，ベタメタゾンの点眼を開始した。2か月後に硝子体混濁が増加し，右眼血管アーケードの上方に0.5乳頭径大の網膜裂孔が発見された。裂孔周辺には網膜血管炎によると思われる出血斑がみられ，裂孔の形成にぶどう膜炎の関与が疑われた。網膜裂孔周囲に光凝固を行い，以後の経過は良好である。結論：眼サルコイドーシスにおいて網膜裂孔を生じることがあり，それによる網膜剝離の可能性にも留意する必要がある。

Purpose：To report a case of retinal tear secondary to ocular sarcoidosis. Case and Findings：A 60-year-old woman presented with seeing floaters. Corrected visual acuity was 1.0 right and 0.7 left. Both eyes showed signs of iridocyclitis,retinal exudate,and retinal vasculitis. Biopsy of skin nodule led to the diagnosis of sarcoidosis. Two months after topical treatment with betamethasone,the right eye developed a retinal tear associated with hemorrhagic patches. Retinal hemorrhage was observed beside the break suggesting the presence of perivasculitis and uveitis as a cause of it. Photocoagulation was performed around the tear with favorable outcome. Conclusion：This case suggests the possibility of developing retinal breaks and further retinal detachment in the sarcoidosis patients.
