一次縫合がなされなかった眼球破裂の3例

Three cases of eyeball rupture treated by vitrectomy with no previous sutures

目的：一次縫合をしないで硝子体手術を行った眼球破裂3例の報告。症例と経過：症例は鈍的外傷で眼球破裂を起こした35歳男性，50歳女性，56歳男性である。初診時の視力は3例とも手動弁であった。結膜と角膜に損傷はなく，全例に硝子体出血があった。初期治療として一次縫合をせずに創傷部の自然閉鎖を待ち，受傷後19日，34日，45日に硝子体手術を行った。3例とも増殖性硝子体網膜症が発症していたが，複数回の手術で網膜が復位し，最終矯正視力として0.1，0.7，0.1が得られた。結論：眼球破裂では一次縫合を行うことが原則であるが，一定の条件が満たされれば創傷部が自然閉鎖したうえで硝子体手術をすることで良好な結果が得られる。

Purpose：To report three cases of eyeball rupture treated by vitrectomy with no previous sutures. Cases：This report concerns two males and one female aged 35, 50 and 56 years respectively. They showed no injury in the conjunctiva or cornea. All had vitreous hemorrhage. They received no primary suture to the site of eyeball rupture. After spontaneous closure of the wound, vitrectomy was performed on day 19, 34, and 45 after injury. All had developed proliferative vitreoretinopathy（PVR）. The retina was reattached after multiple surgeries. Final corrected visual acuity was 0.1, 0.7 and 0.1 respectively. Conclusion：Primary suture of the wound is advocated for eyeball rupture. Good outcome is expected by performing vitrectomy with no primary suture in cases that satisfy certain conditions.
