Abstract. Purpose:To report a child of macular hypoplasia who was initially diagnosed with idiopathic amblyopia. Case:A 5-year-old boy presented with impaired visual aciuty in the right eye. The corrected visual acuity was 0.5 right and 1.0 left. He had intermittent exotropia with simultaneous binocular vision. Clinical Course:He was diagnosed with idiopathic amblyopia and received occlusion of left eye. Visual acuity in the right eye was 0.9 after 2 months, 0.9 after 2 years, and 0.4 after 6 years. Detailed examination of the fundus showed absence of foveolar and ring reflex. Fluorescein angiography showed absence of avascular fovea. Optical coherence tomograph(OCT)showed flat fovea lacking foveal depression. These findings led to the diagnosis of macular hypoplasia. Conclusion:Detailed fundus examination is necessary to rule out organic lesions in suspectd cases of idiopathic amblyopia.
