要約 目的:新規に導入された眼科救急体制下での診療状況の報告。対象と方法:市立三次中央病院では,眼科の救急患者の診療を最初から眼科医が行っていた。これを2005年9月から当直医がまず診療する体制に変わった。新体制の導入から7か月間に受診した眼科救急患者を統計的に検討した。結果:眼科に関係する214名の患者が受診した。うち107名について当直医から眼科医に診療の要請があった。これら疾患の多くには,外傷歴,霧視,眼痛があった。当直医は角膜や結膜異物の確認に苦慮していた。結論:眼科の救急患者を当直医がまず診療する体制下では,プライマリケアを充実させるために,対応マニュアルを作成する必要がある。
Abstract. Purpose:To report the new on-duty system to treat emergency eye patients in the Miyoshi Central Hospital. Objects and Method:Under the old system,emergency eye patients were seen and treated by an ophthalmologist. Since September 2005,they were first seen by a physician on duty and were referred to an ophthalmologist when necessary. Eye patients were reviewed who were treated during 7 months after introduction of the new system. Results:A total 214 patients sought medical care for eye problems. The physician on duty asked the ophthalmologist for help on 107 patients. Most of the patients had history of trauma,blurring of vision,or ocular pain. The physician on duty had difficulties to locate foreign bodies of the cornea or the conjunctiva. Conclusion:It is necessary to provide a manual for management to improve the primary care,when emergency eye patients are to be seen first by the phsician on duty.
