Abstract. We investigated the current status of critical pathway in cataract surgery. A questionnare was sent to 10 institutions belonging to a policy-based medical services network throughout Japan. These institutions reviewed their clinical records and asked patients regarding the in-hospital care. Critical pathway wasadopted in 8 institutions. A total of 236 patients responded to the study.Their ages averaged 71.2 years.

There was no significant difference in the duration of hospital stay or in the medical cost dependent on the use of critical pathway. Subjective evaluation including satisfaction of patients did not differ dependent on the use of critical pathway. The findings show that introduction of critical pathway does not positively affect the clinical functions in medical institutions. We advocate modification in the current status by setting explicit goals based on the need of patients.
