要約　白内障手術でのクリティカルパスの導入状況と，診療に対する患者からの評価を調査した。国立病院・療養所感覚器政策医療ネットワーク10施設を対象とし，診療録調査と患者アンケートによる横断調査を行った。クリティカルパスは8施設で導入され，患者数は236名，年齢は平均71.2歳であった。クリティカルパスの有無は，平均入院日数とレセプト点数に有意に関係しなかった。患者アンケートによる主観的評価と満足度は，両群間に差がなかった。現時点では，10施設でのクリティカルパスによる臨床機能への有効性は確認できず，より明確に主観的評価に対するアウトカムを設定し，患者に立脚したクリティカルパスの改善が望まれる。

Abstract.　We investigated the current status of critical pathway in cataract surgery. A questionnare was sent to 10 institutions belonging to a policy-based medical services network throughout Japan. These institutions reviewed their clinical records and asked patients regarding the in-hospital care. Critical pathway wasadopted in 8 institutions. A total of 236 patients responded to the study.Their ages averaged 71.2 years.

There was no significant difference in the duration of hospital stay or in the medical cost dependent on the use of critical pathway. Subjective evaluation including satisfaction of patients did not differ dependent on the use of critical pathway. The findings show that introduction of critical pathway does not positively affect the clinical functions in medical institutions. We advocate modification in the current status by setting explicit goals based on the need of patients.
