頚椎椎弓形成術クリニカルパスのアウトカム評価

Evaluation of the Clinical Pathway for Cervical Laminoplasty

　頚椎椎弓形成術クリニカルパス(以下，パス)を使用した85例におけるアウトカムの達成率を調査し，パスの妥当性を評価した．最終アウトカム(術後14日以内に自宅へ退院)の達成率は47％であり，術後入院期間の中央値も設定と同じ14日であることから，パスの設定としては妥当であると考えられた．最終アウトカムを達成できなかったバリアンス症例への対応には，パスの適応基準を細分化する方法と入院経過中に最終目標を変更するための指標を設定する方法の二つがあるが，後者がより実際的な対応であると考えられた．

　We investigated the outcome achievement rate in 85 cases treated according to a clinical pathway for cervical laminoplasty, and evaluated the validity of the clinical pathway.　The final outcome achievement rate (discharge from hospital within 14 days postoperatively) was 47%, and the median postoperative hospital stay was 14 days.　Thus, the setting of the clinical pathway was considered to be valid.　We considered 2 methods to correspond to the variance cases in which the final outcome was not achieved：1) to segmentalize the indication criteria of the clinical pathway, and 2) to set the index to change the final target during the patient's hospital course.
