Abstract. We performed vitrectomy for idiopathic dense vitreous hemorrhage in 11 eyes of 11 patients during the past 5 years. The age of patients ranged from 44 to 92 years,average 72 years. The interval between onset and surgery ranged from 4 to 730 days,average 117 days. Cases were followed up after surgery for 90 to 390 days,average 206 days. The intraoperative diagnosis comprised retinal break with or without retinal detachment 4 eyes,retinal arteriolar macroaneurysm 3 eyes,branch retinal vein occlusion 3 eyes,and age-related macular degeneration 1 eye. Eyes of retinal break with or without retinal detachment resulted in good visual outcome due to early surgery. Eyes of longstanding subretinal hemorrhage resulted in poor visual outcome. We advocate early vitrectomy for idiopathic dense vitreous hemorrhage within one month of onset regardless the final diagnosis.
