角膜内皮障害を伴う白内障に対する経毛様体扁平部水晶体摘出術

Pars plana lensectomy for cataract with corneal endothelial damage

　角膜内皮細胞密度が1,500個／mm²以下の白内障3例3眼に経毛様体扁平部水晶体摘出術を行った。2眼には眼内レンズ挿入を行い，毛様溝固定とした。年齢はそれぞれ67歳，80歳，92歳であった。全例で視力が改善し，角膜内皮細胞密度には術前後で変化はなかった。術中合併症として1眼で皮質落下があった。経毛様体扁平部水晶体摘出術では前嚢が温存されるので，水晶体乳化の際の超音波エネルギーや灌流液の前房への影響が少なく，角膜内皮障害を防止すると考えられた。この方法は，角膜内皮障害がある白内障への有効な術式であると結論される。

We performed pars plana lensectomy on 3 eyes of 3 cases of cataract with corneal endothelial density below 1,500/mm². Two eyes received intraocular lens implantation fixated in the sulcus. The patients were aged 67, 80 and 92 years respectively. The visual acuity improved in all the eyes. There was no change in the corneal endothelial cell population after surgery. As intraoperative complication, the lens cortex fell into the vitreous in one eye. The anterior capsule, which is kept intact in this procedure, would prevent further corneal endothelial damage due to irrigating solution or ultrasound energy. It is concluded that pars plana lensectomy is useful for cataract with pre-existing corneal endothelial damage.
