We performed pars plana lensectomy on 3 eyes of 3 cases of cataract with corneal endothelial density below 1,500/mm2. Two eyes received intraocular lens implantation fixated in the sulcus. The patients were aged 67, 80 and 92 years respectively. The visual acuity improved in all the eyes. There was no change in the corneal endothelial cell population after surgery. As intraoperative complication, the lens cortex fell into the vitreous in one eye. The anterior capsule, which is kept intact in this procedure, would prevent further corneal endothelial damage due to irrigating solution or ultrasound energy. It is concluded that pars plana lensectomy is useful for cataract with pre-existing corneal endothelial damage.
