Abstract Purpose: To report a case who developed anaphylactic shock after intravenous minocycline prior to vitreous surgery.

Case: A 72-year-old male presented with impaired vision in the left eye since one month before. The left eye showed vitreous hemorrhage and was scheduled for vitreous surgery. He received intravenous infusion of minocycline in the surgical room. He started coughing and nausea 6 minutes after start of infusion. He had apparently received 30 mg of minocycline. He developed skin rashes and decrease in blood pressure followed by cardiopulmonary arrest. He was resuscitated after electric defibrillation. His general status improved after treatment in the intensive care unit.

Conclusion: This case illustrates that intravenous infusion of minocycline may induce anaphylactic shock. Prompt identification of anaphylaxis and initial treatment were essential for recovery.
