術前抗菌薬投与にてアナフィラキシーショックから心肺停止に至った硝子体出血の1例

A case of anaphylactic shock after intravenous minocycline prior to vitreous surgery

要約　目的：硝子体手術の術前投与として，ミノサイクリンの全身投与により，アナフィラキシーショックから心肺停止に至った1例を報告する。

症例：72歳男性。主訴は左視力低下。左眼に原因不明の硝子体出血を発症し，診断および加療目的で硝子体手術を予定した。手術室にてミノマイシン点滴静注開始後，咳嗽，悪心，発疹が出現。さらに血圧低下から意識消失し，蘇生処置を開始するも，心室頻拍から心肺停止となった。電気的除細動を施行し，心電図波形が改善した。ICUへ移送し，数日後には全身状態は安定した。

結論：眼科手術でも抗菌薬予防的投与によるアナフィラキシーショックの発症は避けられず，早期診断，救急処置，他施設への移送を含め，緊急時に対する準備が必要である。

Abstract　Purpose: To report a case who developed anaphylactic shock after intravenous minocycline prior to vitreous surgery.

Case: A 72-year-old male presented with impaired vision in the left eye since one month before. The left eye showed vitreous hemorrhage and was scheduled for vitreous surgery. He received intravenous infusion of minocycline in the surgical room. He started coughing and nausea 6 minutes after start of infusion. He had apparently received 30 mg of minocycline. He developed skin rashes and decrease in blood pressure followed by cardiopulmonary arrest. He was resuscitated after electric defibrillation. His general status improved after treatment in the intensive care unit.

Conclusion: This case illustrates that intravenous infusion of minocycline may induce anaphylactic shock. Prompt identification of anaphylaxis and initial treatment were essential for recovery.
