Abstract. We reviewed the outcome of penetrating keratoplasty using directly excised corneoscleral button from the donor eye in 14 cases. The excision was performed with a portable electric trephine,Micro-KeratonTM. The transplanted cornea remained transparent in 50%,slightly opaque in 21%,and opaque in 29%. The visual acuity improved in 57%,remained unchanged in 36%,and deteriorated in 7%. Graft rejection occurred in 21%. In another series of 10 cases,transplanted cornea from enucleated eyeball remained transparent in 30% and visual acuity improved in 60%. In a third series of 75 cases,transplanted cornea from preserved eyeball remained transparent in 42% and visual acuity improved in 42%. The findings show that the use of portable electric trephine can be recommended for penetrating keratoplasty whenever feasible.
