左同名半盲を呈したエタンブトール視神経症の1例

Left homonymous hemianopia in a case of ethambutol neuropathy

（P2-2-1）　エタンブトール視神経症患者が左同名半盲を呈し，重度の視野障害を残した。症例は60歳の男性でエタンブトール投与後，約50日目から視野障害を自覚し，当科へ紹介された。両眼とも矯正視力は0.5，軽度の白内障と傾斜乳頭があり，視野検査で左同名半盲を呈した。MRIでは頭蓋内には明らかな異常はなかった。その後，左視力が0.2と低下したため，エタンブトール視神経症と診断し，エタンブトール投与を中止した。1週間後，左視力は回復したが，視野欠損は改善しなかった。6か月後，視野はわずかに改善したが，その後の視野検査で変化はなかった。

A 60-year-old male presented with visual field disturbance. He had been under systemic treatment with ethambutol for miliary tuberculosis, totalling 63g. His visual acuity was 0.5 in either eye. The fundus appeared to be practivally normal. Perimetry showed left homonymous hemianopia. No intracranial disorder was detected by magnetic resonance imaging. The left visual acuity deteriorated to 0.2 three weeks later, leading to the diagnosis of ethambutol neuropathy. The visual acuity improved to 0.6 one week after discontinuation of ethambutol. The visual field defect remained almost stationary for 10 months later.
