　34歳男性が右眼の硝子体出血で受診した。右眼の矯正視力は0.03で，眼底に網膜下出血と黄白色の滲出性病変があった。全身検査で，Ehlers-Danlos症候群（Ⅱ型），後頭部に感染性粉瘤，腎に両側性の嚢胞性病変が発見された。血清の抗リン脂質抗体が陽性であった。硝子体切除術後，Ⅱ型脈絡膜新生血管が中心窩の近傍にあった。以後通院が中断したが，約11か月後の左腎破裂の際に右眼に全網膜剥離が生じていた。腎嚢胞から黄色ブドウ球菌が検出され，病理学的に黄色肉芽腫性腎盂腎炎と診断された。腎摘出後に左眼に網膜色素上皮障害が生じた。脈絡膜新生血管に多数の要因が関係することを示す症例である。

A 34-year-old man was referred to us for vitreous hemorrhage in his right eye with visual acuity of 0.03.Funduscopy showed numerous hemorrhagic patches and subretinal exudates. Systemic studies showed Ehlers-Danlossyndrome, infectious atheroma, bilateral renal cystic lesions, and elevated serum level of antiphospholipid antibody.Following vitrectomy, choroidal neovascularization was found in the parafoveal region. When seen again for rupture ofhis left kidney, total retinal detachment was present. Histopathology showed xanthogranulomatous pyelonephritis causedby Staphylococcus aureus. Punctate lesions in the retinal pigment epithelium occurred in the left eye after nephrectomy.This case illustrates that choroidal neovascularization may be induced by various factors.
