漿液性網膜剝離を契機として原発性肺癌が発見された転移性脈絡膜腫瘍の1例

Serous retinal detachment as the initial manifestation in a case of lung cancer metastatic to the choroid

要約　目的：漿液性網膜剝離を初発症状とした肺癌と転移性脈絡膜腫瘍の症例の報告。症例：77歳女性が急激な左眼霧視を自覚し，その2日後に受診した。所見：矯正視力は右0.9，左0.2であり，左眼の後極に扁平な漿液性網膜剝離があった。中心性漿液性網脈絡膜症や原田病が疑われたが，その後の精査で原発性肺癌と遠隔転移が発見された。光干渉断層計では網膜下に扁平な隆起病変が認められ，これは化学療法により縮小を示し，肺癌からの脈絡膜転移と診断された。結論：漿液性網膜剝離による視力障害が唯一の臨床症状として，これを契機に未発見の悪性腫瘍が判明することがある。

Abstract.　Purpose：To report a case of serous retinal detachment secondary to choroidal tumor metastatic from occult lung cancer. Case and Findings：A 77-year-old presented with sudden visual impairment in her left eye since 2 days before. Her corrected visual acuity was 0.9 right and 0.2 left. Her left fundus showed flat serous retinal detachment in the macular area. The findings simulated serous central chorioretinopathy or Harada disease. Systemic studies led to detection of lung cancer and metastases. Optical coherence tomography（OCT）showed a flat lesion posterior to the retina. Chemotherapy was followed by diminution of subretinal lesion,suggesting it to be choroidal tumor metastatic from the lung cancer. Conclusion：This case illustrates that serous retinal detachment may be the only manifestation of choroidal tumor metastatic from occult cancer.
