要約　目的：裂孔原性網膜剝離と鑑別を要した転移性脈絡膜腫瘍による滲出性網膜剝離の1例の報告。

症例：51歳，男性。1か月前からの左眼変視症を自覚し近医を受診した。左眼下方の網膜の格子状変性と網膜剝離を指摘され，当院を紹介受診した。受診1年半前にS状結腸癌切徐術を施行され，1か月前に多発肺転移とリンパ筋転移を指摘された。

所見と経過：左眼は後極部の黄白色の隆起性病変を中心に網膜剝離を認め，下方の格子状変性部の網膜剝離と交通していた。蛍光眼底造影検査で後極部に漏出を認め，結腸癌の多発転移に対して化学療法予定であったことから，後極部の網膜剝離は転移性脈絡膜腫瘍による滲出性網膜剝離と診断した。下方の網膜剝離は格子状変性内の小裂孔に起因する裂孔原性の可能性も否定できなかったが，まず化学療法で網膜剝離が減少するか経過観察した。化学療法開始後，網膜剝離は速やかに消退傾向となり，2か月後には消失した。

結論：転移性脈絡膜腫瘍による滲出性網膜剝離は病歴，蛍光眼底造影検査から診断できるが，剝離の範囲に網膜格子状変性部がある場合，裂孔原性網膜剝離の可能性も考えられ，その形態と裂孔の有無を検眼鏡的に正確に評価することが重要と考えられた。

Abstract　Purpose：To report a case of exudative retinal detachment secondary to metastatic choroidal tumor that needed differentiation from rhegmatogenous retinal detachment.

Case：A 51-year-old man noted metamorphopsia in the left eye one month before. He was referred to us for retinal detachment with lattice degeneration in the inferior periphery. He had received surgery for colon cancer 18 months before and metastasis to the lung and lymph nodes was detected one month before.

Findings and Clinical Course：Corrected visual acuity was 1.5 right and 0.5 left. The left eye showed a white mass in the posterior fundus with retinal detachment extending inferiorly. Lattice degeneration was present in the inferior retina. Fluorescein angiography showed multiple leakage in the white mass in the posterior fundus, leading to the diagnosis of exudative retinal detachment secondary to choroidal tumor. The retinal detachment disappeared after 2 months of systemic chemotherapy alone.

Conclusion：Retinal detachment secondary to metastatic choroidal tumor can be diagnosed by medical history and fluorescein angiographic findings. In the present case, response to chemotherapy played a key role in differentiating rhegmatogenous retinal detachment due to lattice degeneration in the inferior periphery.
