裂孔原性網膜剝離術後にみられた眼底多発性斑状変化の3例

Three cases of multifocal patches in the fundus following surgery for rhegmatogenous retinal detachment

要約　目的：網膜剝離手術後に斑状変化が網膜に生じた3症例の報告。症例：裂孔原性網膜剝離が片眼に発症した49歳男性，57歳女性，72歳男性に手術を行った。2例には強膜陥凹術，1例には硝子体手術を行った。3例とも初回手術で復位が得られたが，手術の2週～3か月後に網膜に斑状変化が多発した。2例では術前の剝離部に，1例では非剝離部に生じた。いずれの症例でも斑状変化は自然寛解または消失した。結論：観察された斑状変化は，手術の侵襲で生じた網脈絡膜循環障害が関与している可能性がある。

Abstract.　Purpose：To report multifocal patches in the fundus following retinal detachment surgery. Cases and Findings：The present series comprised 2 males and 1 female. They were aged 49,57 and 72 years. All had rhegmatogenous retinal detachment unilaterally. Scleral buckling was performed on 2 cases. Vitrectomy was performed in 1 case. The retinal became reattached in all the cases. Multifocal patches appeared in the retina 2 weeks to 3 months after surgery. In 2 cases,they were located in previously detached fundus area. In 1 case,the patches were located in non-detached area. The patches disappeared spontaneously in all the cases. Conclusion：The observed multifocal patches may have been the result of disturbed retinochoroidal circulation secondary to surgical insult.
