黄斑前膜に対する硝子体手術成績およびICG染色による術後合併症

Outcome of vitreous surgery for epiretinal membrane and complications of staining with indocyanine green

　黄斑前膜23眼に硝子体手術を行った。17眼は特発性，6眼は続発性であった。16眼では術中にインドシアニングリーン（ICG）で内境界膜を染色した。特発性群の12眼と続発性群の5眼で術後視力が向上した。術前と術後6か月目の視力の比較では，特発性群よりも続発性群での視力向上の程度が大きかった（p＝0.013）。術翌日からの低眼圧と脈絡膜剝離がICG染色群の4眼（25％）に起こったが，ICG非使用例にはなかった。低眼圧と脈絡膜剝離は4眼すべてで1週間以内に自然寛解した。低眼圧が硝子体手術時のICG染色の副作用である可能性と，これに毛様体の房水産生能の低下が関係している可能性がある。

　We reviewed 23 eyes that received vitreous surgery for epiretinal membrane. It was idiopathic in 17 eyes and secondary in 6. The internal limiting membrane was stained by indocyanine green（ICG）in 16 eyes. After surgery,the visual acuity improved in 12 eyes of idiopathic group and in 5 eyes of secondary group. When compared with preoperative level,visual acuity 6months after surgery was significantly better in secondary group than in idiopathic group（p=0.013）. Ocular hypotony with choroidal detachment developed on day 2 of surgery in 4（25％）of ICG-stained eyes. Spontaneous cure set in within one week in all the 4 eyes. There is a possibility that ocular hypotony was causatively related with ICG staining and that it was secondary to decreased aqueous production in the ciliary body.
