黄斑前膜の硝子体手術に影響する因子の検討

Factors affecting the outcome of vitrectomy for epimacular membrane

　1991年11月から1999年4月までの期間に，黄斑前膜症例に行った硝子体手術の成績を術前後の視力変化，自覚症状の存在期間，膜の性状，蛍光漏出の有無などの因子について比較した。対象は硝子体手術を施行し，手術後6か月以上経過観察できた特発性黄斑前膜35例37眼，続発性黄斑前膜20例20眼の計55例57眼である。統計学的に有意差はみられなかったが，術前視力が良好で自覚症状のある期間が明らかに短いもののほうが，術後視力は良好であった。膜の性状では，セロファン状のもの，さらに術前の蛍光眼底検査で，網膜からの蛍光漏出のない症例のほうが視力改善が良好であった。本症に対する手術の適応については，術前視力は0.4もしくはそれ以上で，膜の混濁する前で蛍光漏出のない症例に対しての，いわゆる早期手術によってよりよい術後視力が得られるものと思われた。

We reviewed 57 eyes which received vitreous surgery for epimacular membrane. The epimacular membrane was idiopathic in 37 eyes and secondary in 20 eyes. When evaluated more than 6 months of surgery, final visual acuity was better in eyes with better visual acuity at the time of surgery and in eyes with shorter duration of the disease. It was also better when the epimacular membrane was transparent simulating cellophane maculopathy and when fluorescein angiography did not show dye leakage. It is concluded that the outcome of vitreous surgery is better when the presurgical visual acuity is 0.4 or more, when the epimacular mambrane is still transparent, and when fluorescein angiography shows no dye leakage.
