眼窩隔膜を利用した腱膜修復手術の手術成績

Outcome of surgery for blepharoptosis using the orbital septum as substitute for aponeurosis

　眼瞼下垂に対して，眼瞼挙筋の腱膜を瞼板に縫着する腱膜修復手術がある。眼窩隔膜を利用する腱膜修復手術を27例51眼瞼に行った。眼瞼下垂の原因は，老人性22例，コンタクトレンズ性3例，内眼手術後2例であった。手術による眼瞼挙上効果は平均4.66±1.54mmで，術後の瞼裂幅は平均8.47±1.31mmと，ともに良好であった。合併症と再発はなかった。本術式で良好な開瞼効果が得られたのは，眼瞼挙筋機能が回復したためと，挙筋とミュラー筋の連動が再構築されたためと解釈される。

　Some types of blepharoptosis may improve by suturing the aponeurosis of levator muscle to the tarsus. We used orbital septum as substitute for apeoneurosis on 51 eyelids of 27 patients. Blepharoptsis was secondary to aging in 22 patients,to contact lens in 3,and to previous intraocular surgery in 2. Outcome of surgery was judged as good,as the upper eyelid was elevated by an average of 4.66±1.54mm and the height of eyelid opening averaged 8.47±1.31mm. The observed surgical effect was due to functional recovery of levator muscle as well as to reestablished co-ordination between the levator and Muller muscles.
