核硬度別に超音波出力の上限を設定したオシレーション付加超音波白内障手術装置(ネオソニックスTM)の核乳化吸引効率を検討した。摘出豚眼で水晶体核を吸引し,水晶体囊にカートンNTMを注入して模擬核を作製した。核の硬度はEmery-Little分類のG2またはG3とした。超音波出力はG2では15%,G3では20%とし,4分割核を上限設定作動(オン)と非作動(オフ)で乳化吸引するときの超音波作動時間と乳化吸引時間を各5回測定した。G2の模擬核での超音波作動時間の平均は,オンで17.5秒,オフで25.8秒であり,乳化吸引時間の平均は,オンで18.3秒,オフで31.3秒であった。G3の模擬核での超音波作動時間の平均は,オンで22.0秒,オフで29.0秒であり,乳化吸引時間の平均は,オンで27.0秒,オフで33.5秒であった。核硬度別に超音波出力の上限を設定した超音波白内障手術装置は,超音波作動時間と乳化吸引時間を短縮する。
Purpose:To evaluate the efficacy of phacoemulsification-aspiration(PEA)instrument(Neo-SonixTM)with power output limiter for different hardness of lens nucleus. Method:Mock lens nucleus was produced in pig eyes. The hardness was set at either G2 or G3 according to classification by Emery-Little. We measured the duration of ultrasound application and PEA. The ultrasound output was set at 15%for G2 and at 20%for G3 lens nucleus. Results:Ultrasound application for G2 averaged 17.5 sec when the limiter was on and 25.8 sec when off. PEA time averaged 18.3 sec when the limiter was on and 31.3 sec when off. Ultrasound application for G3 averaged 22.0 sec when the limiter was on and 29.0 sec when off. PEA time averaged 27.0 sec when the limiter was on and 33.5 sec when off. Conclusion:PEA instrument with power output limiter for different hardness of lens nucleus reduced the duration of ultrasound application and PEA under experimental setting.
