 We reached the following therapeutic guidelines after reviewing three cases of retinal macroaneurysm. All these cases failed to show increase of retinal hemorrhage. Vitreous surgery is to be refrained for one month at least if initial hemorrhage does not increase. Photocoagulation is to be preferred if the macroaneurysm is accompanied by hard exudates or hemorrhage or if pulsation is observed by indocyanine green angiography. Persistent hemorrhage is to be treated by vitreous surgery including peeling of internal limiting membrane.
