A 59-year-old male developed convulsion. He was hospitalized under the diagnosis of atrial flutter,multiple cerebral infarction,and obliterating arteriosclerosis. He was referred to us for eye examinations 2 weeks later. Both eyes had normal visual acuity,intraocular pressure,pupil reactions,and critical flicker fusion frequency(CCF). Both eyes showed papilledema. Elevated intracranial pressure appeared unlikely,as diagnostic imaging showed no space-occupying lesiosn or enlarged ventricles. Clinical symptoms related to papilledema were also absent. When lumbar puncture was performed 6 weeks later,the initial cerebrospinal pressure was 300mm H2O. Chemical composition of the fluid was normal. Later examinations led to detection of a pseudocerebral tumor secondary to thrombosis of the superior sagittal sinus. This case illustrates that pseudocerebral tumor may induce papilledema in the absence of related clinical manifestations or enlarged ventricles.
