要約　目的：うっ血乳頭を契機として脳静脈洞血栓症が発見された症例の報告。症例：36歳男性が10日前からの頭痛と変視症でうっ血乳頭を発見され，紹介受診した。矯正視力は左右とも1.0で，両眼に顕著なうっ血乳頭があった。他に眼科的に異常はなかった。コンピュータ断層撮影（CT）では異常所見はなかった。脳神経外科での磁気共鳴画像静脈検査（MRV）で脳静脈洞血栓症と診断された。抗凝固療法で血栓は溶解し，治療開始から2週間後に頭痛は消失し，6か月後にうっ血乳頭は改善した。結論：うっ血乳頭がCTでは検出できない脳静脈洞血栓症で発症する可能性がある。

Abstract.　Purpose：To report a case of cerebral venous sinus thrombosis with bilateral papilledema as the initial manifestation. Case：A 36-year-old male was referred to us for papilledema in both eyes following headache and metamorphopsia since 10 days before. Visual acuity was 1.0 in either eye. Marked papilledema was present in both eyes. He was free from other ophthalmological disorders. Findings by computed tomography（CT）were inconclusive. Magnetic resonance venography（MRV）led to the diagnosis of cerebral venous sinus thrombosis. Anticoagulant therapy was followed by dissolution of thrombus. Headache disappeared 2 weeks later. Papilledema disappeared 6 months after onset of treatment. Conclusion：This case illustrates that cerebral venous sinus thrombosis may not be detected by conventional CT and that it may induce papilledema in both eyes.
