We observed three cases of idiopathic orbital myositis with different outcomes after systemic corticosteroid therapy. All were females aged 42, 47 and 39 years respectively. One acute unilateral case improved promptly after systemic corticosteroid. One chronic unilateral case resulted in persistent oculomotor restriction after three courses of systemic corticosteroid pulse therapy. One acute bilateral case improved after a pulse therapy but recurred 4 months later. These cases of orbital myositis illustrate that a high-dose systemic corticosteroid is important during the early stage of the disease with very gradual tapering of the dose. Corticosteroid pulse therapy appears to be indicated for bilateral involvement.
