眼窩筋炎の3例

Three cases of orbital myositis

　ステロイド薬による治療後，異なる経過をとった眼窩筋炎の3症例を経験した。いずれも女性であり，年齢は，42歳，47歳，39歳である。片眼性の急性期の1例は，ステロイド治療後に寛解した。片眼性の慢性期の1例は，ステロイドパルス療法後も眼球運動障害が残った。両眼性の急性期の1例は，ステロイドパルス療法後に軽快したが4か月後に再発した。眼窩筋炎の治療では，発症早期に十分量のステロイド薬を投与すること，その漸減を急がないことが重要であること，両眼性の症例にはステロイドパルス療法が望ましいことが推論される。

We observed three cases of idiopathic orbital myositis with different outcomes after systemic corticosteroid therapy. All were females aged 42, 47 and 39 years respectively. One acute unilateral case improved promptly after systemic corticosteroid. One chronic unilateral case resulted in persistent oculomotor restriction after three courses of systemic corticosteroid pulse therapy. One acute bilateral case improved after a pulse therapy but recurred 4 months later. These cases of orbital myositis illustrate that a high-dose systemic corticosteroid is important during the early stage of the disease with very gradual tapering of the dose. Corticosteroid pulse therapy appears to be indicated for bilateral involvement.
