Abstract. A 65-year-old man was hit by a foreign body in his left eye during mowing. When he was seen 24 hours later,his left visual acuity was hand motion. The left eye was protruding with swollen eyelids. A perforating wound was present in the cornea. He was diagnosed with intraocular foreign body and endophthalmitis. During the few hours before surgery,his left cornea developed rapid opacity. A ferrous foreign body was present in the removed lens.The affected eye received additional anterior vitrectomy. Bacillus cereus was isolated from the intraocular specimen. Systemic antibiotics including vancomycin induced remission 12 days later. The affected eye became hypotonic with the visual acuity of hand motion. This case illustrates that endophthalmitis due to Bacillus cereus may run a fulminant course needing immediate treatment.
