Purpose:To report massive migration of indocyanine green(ICG)into the vitreous during cataract surgery. Case:A 52-year-old male presented with cortical cataract in his right eye. His corrected visual acuity was 0.03 right. He was scheduled for cataract surgery. Results:During surgery,the anterior chamber was filled by viscoelastic material followed by intracameral injection of ICG solution. After irrigation of anterior chamber,phacoemulsification-aspiration was performed. Dark appearance of the pupil led to detection of ICG in the vitreous. Vitrectomy was performed to completely remove the ICG. There was no staining of the retina by the dye. The visual acuity improved to 1.5 three days after surgery. There has been no problem regarding clinical findings,visual field,or electroretinogram during the ensuing 18 months. Conclusion:Migration of ICG into the vitreous did not induce pathological changes in the eye for 18 months. It is estimated that 0.5 mg of the dye entered the vitreous.
