We performed a second vitrectomy on two eyes with adjunct use of indocyanine green (ICG) staining. In one eye of diabetic macular edema persisting after the initial vitrectomy, ICG staining showed a vague ring along the vascular arcades. This ring corresponded to the posterior wall of posterior precortical vitreous pocket (PPVP). Guided by this finding, we performed detachment of the posterior vitreous and the internal limiting membrane (ILM). In another eye in which the macular hole reopened 4 years after previous vitrectomy, ICG staining showed the area of ILM detachment and the presence of proliferative membrane along its margin. Guided by this finding, we detached the remaining ILM. ICG staining was thus useful in assessing the outcome of previous vitrectomy and in deciding the surgical modality during the second vitrectomy.
