　後縦靱帯骨化は脊髄を前方より圧迫し，一般に胸椎部は後弯であることから，胸椎後縦靱帯骨化症の観血的治療は前方除圧が合理的であり，当科では胸骨縦割アプローチまたは経胸膜進入による前方除圧術を第1選択としている．当科での胸椎後縦靱帯骨化症に対する前方除圧術を行った20例の成績につき調査した．画像的に良好な除圧が得られており，手術成績も平均改善率60％と良好であった．前方進入前方除圧術はアプローチ，手術椎間数など一部に制約があるものの手術効果の点で有効な術式である．
