下肢整形外科手術施行患者におけるエノキサパリンナトリウムの有用性

Usefulness and Safety of Enoxaparin in Patients with Orthopaedic Surgery for Lower Extremities

　エノキサパリンは，アンチトロンビンⅢ(ATⅢ)と複合体を形成し，ATⅢの第Xa因子活性と第Ⅱa因子活性を阻害することにより抗凝固作用を発現する低分子量ヘパリンである．日本人の下肢整形外科手術施行患者の静脈血栓塞栓症予防に対するエノキサパリンの効果を確認するために，ランダム化二重盲検比較試験3試験と単一群非盲検試験1試験を実施し，エノキサパリン20mg 1日2回投与(術後24～36時間に投与開始)の有効性および安全性が示された．

　Enoxaparin is a low-molecular-weight heparin which forms a complex with antithrombin III (AT III) and exerts its anticoagulant activity through AT III, an endogenous inhibitor of factor Xa and factor IIa. (thrombin).　Three randomized, double-blind, studies and one open-label single arm study were conducted to confirm the efficacy and safety of enoxaparin for prevention of lower limbs orthopedics operation in Japan, and the propriety of enoxaparin 20mg bid (administered 24～36 hours postoperatively) was demonstrated.
