テント上脳血管障害後のWaller変性と機能予後

Wallerian Degeneration Demonstrated by MRI and Functional Outcome in Patients Suffering from Supratentorial Cerebrovascular Disease

　〈目的〉MRI上のWaller変性および中脳萎縮と臨床予後との関係を検討した。〈方法〉片麻痺をきたし，発症後1日から6,275日にMRIを撮影されたテント上脳出血50例，脳梗塞37例，計87例を対象とした。経過中36例は複数回撮影をし，合計161の画像を得た。MRI水平断T2強調画像にて2 slice以上にわたり錐体路に高信号を認めたものをWaller変性とした。また，脳幹萎縮度は（健側中脳面積一病変側中脳面積）／（健側中脳面積×2）により算出した。発症1年以後の時点で観察できた64症例に対し臨床的予後をBarthel Indexで判定し信号変化および萎縮度と臨床予後との関係を分析した。2回以上の撮影が行われた36例についてWaller変性と萎縮度の経時的変化を分析した。〈結果〉経過中に高信号を認めたものは32例（37％），58撮影（36％）であり，発症後約2カ月から3カ月で出現し，約3年で不明瞭となった。発症後6カ月以内に高信号変化を認めた場合，臨床予後不良であった（p＜0.05）が，それ以後の時期では有意差は認めなかった。また，予後不良例では6カ月以内の早期に萎縮が顕著化し，脳幹萎縮度と予後に相関を認めた（p＜0.001）。一方，それ以後の時期では脳幹萎縮度と臨床予後に相関は認めなかった。〈結論〉脳血管障害発症後，約半年間はMRIによるWaller変性の検索により予後予測がある程度可能であった。

An early diagnosis of the outcome of patients with cerebrovascular disease is important for selecting the optimal treatment strategy. The purpose of this study was to estimate the prognosis of Wallerian degenera-tion on MRI in stroke patients with hemiparesis. The subjects consisted of 87 stroke patients, 50 hemor-rhagic patients and 37 ischemic patients, who were evaluated by MRI at 1 to 6,275 days after stroke onset. Among the 36 patients who were evaluated by con-secutive MRI, 161 films were obtained and analyzed.
