遷延性植物状態をきたした対称性大脳脚梗塞の1例

A Case of the Persistent Vegetative State Following Symmetrical Peduncular Infarction

　症例は64歳女性。突然発症の躯幹の左への偏椅，構音障害で入院した。翌日症状が悪化しMRIで延髄，橋に小梗塞出現が認められた。脳血管撮影で吻側部脳底動脈狭窄を認め，急速に意識障害，弛緩性四肢麻痺に陥った。MRIで大脳脚を中心とする対称性中脳梗塞が描出され，発生機序として脳底動脈遠位部閉塞が推察された。発症後4週間で遷延性植物状態（PVS）をきたし，1年経過後も症状の改善はみられなかった。PVSは広範なテント上病変あるいは両側視床病変で生じることはよく知られているが，本症例は中脳を中心とする脳幹部梗塞でPVSをきたした点で極めて特異的である。

A 64-year-old woman presented with sudden onset of disequilibrium and dysarthria. Two days later she became lethargic and tetraparetic. An emergency MRI disclosed multiple infarctions in the medulla oblongata and pons. An angiogram revealed severe stenosis of the rostral basilar artery. The patient underwent a rap-idly progressive course with impaired consciousness and flaccid tetraplegia. MR images 7 days after the on-set disclosed a symmetrical midbrain infarction that was localized mainly in the cerebral peduncles, sug-gesting a distal basilar artery occlusion as the cause of this pathology.
