　症例は51歳の男性で，交通外傷にて当院に搬送された。来院時意識レベルはGlasgow Coma Scale 6点で左動眼神経麻痺および左不全片麻痺を認めた。頭剖CTで外傷性くも膜下出血を認めた。受傷翌日の頭部MRIで中脳の左背外側および右大脳脚に高信号病変を認め，びまん性軸索損傷と診断した。意識障害は遷延し，四肢の固縮・無動により次第に関節の拘縮をきたして四肢麻痺となり，慢性期に「植物状態」と診断された。受傷7カ月目に外傷性パーキンソン症候群の合併の可能性を考慮しL-dopaの投与を開始したところ，意識状態・運動機能ともに著しい改善を認めた。本例は脳幹損傷によるドーパミン作動系の障害を本態とする外傷性パーキンソン症候群を合併していたと考えられた。植物状態を呈する症例の治療に際しては，本例のように治療によって改善が期待できる"偽性"植物状態の可能性を念頭におく必要があると思われた。

A 51-year-old male was transferred to our hospital just after traffic accident. On admission, the patient was comatose (Glasgow Coma Scale of 6) and showed a left hemiparesis with a left oculomotor nerve palsy. Computed tomography demonstrated a traumatic subarachnoid hemorrhage without mass lesion. Mag-netic resonance imaging showed high intensity lesions on the left dorsolateral midbrain and the right cere-bral peduncle. The distribution of lesions implied dif-fuse axonal injury involving dopaminergic systems such as the substantia nigra and the ventral tegmental area.
