非侵襲的検査で前側頭葉切除術を行った内側側頭葉てんかん

Non-invasive Examinations Successfully Select Patients with Medial Temporal Lobe Epilepsy for Anterior Temporal Lobectomy

　慢性硬膜下電極記録などの侵襲的検査を行わずに前側頭葉・海馬切除術を行った難治性の内側側頭葉てんかん患者8例の臨床像，電気生理学的所見，神経放射線学的所見，術後成績を検討した。5例は熱性けいれんの既往があり，発作時には口部自動症が全例でみられたが，一側肢の自動症と対側肢のジストニア様肢位がみられたのは2例であった。発作間歇期脳波では半数で両側に突発性異常波がみられたが，全例で発作時脳波で一側前側頭部に発作起始が確認できた。MRIで一側に明らかな海馬硬化がみられたものは5例であった。発作間歓期の糖代謝PET（FDG-PET）では全例で一側側頭葉の低代謝がみられたが，脳血流SPECTでは1例で側方性の診断ができなかった。術後7例はseizure free，1例はrare seizureとなった。臨床発作型，電気生理学的所見，神経放射線学的所見がすべて矛盾せず，一側に海馬硬化症が存在することを示唆すれば，侵襲的検査を省略して手術を行ってきたが，特に発作症状に口部自動症があり，発作時脳波とFDG-PET所見の側方性が一致することが手術側を決定する上で参考となった。

We retrospectively analyzed 8 patients with intracta-ble medial temporal lobe epilepsy (MTLE) who under-went the anterior temporal lobectomy with hippocam-pectomy (ATL) without invasive examinations such as chronic subdural electrode recording. Five patients had a history of febrile convulsion. While all 8 patientshad oral automatism, automatism of ipsilateral limbs with dystonic posture of contralateral limbs was dem-onstrated in 2 patients. Bilateral temporal paroxysmal activities on interictal EEG was observed in 4 patients and all patients had clear ictal onset zone on unilateral anterior temporal region.
