　頭部を左側に回転させると視野障害，意識障害が出現するBow hunter's strokeの25歳，男性例を報告した。左後方を振り向くと視野が周囲より中心に向かって狭まり，ついには眼前暗黒となり意識消失する発作が数回みられた。頭部CT，頸部レントゲンでは異常を認めず，血管造影では左椎骨動脈低形成で右椎骨動脈は頭部正中位では異常を認めなかったが，頭部を左に回転させるとC1-2レベルで閉塞した。患者は経験的に発作を予兆できるようになっており，日常生活にも支障ないため経過観察している。若年性のBow hunter'sstrokeは稀であり，その病態，治療方針などにつき文献的考察を加えた。

Bow hunter's stroke results from vertebrobasilar in-sufficiency caused by mechanical occlusion or stenosis of the vertebral artery at the C 1-2 level on head rota-tion. Commonly it is seen in elder people with cervical spondylosis. Here we reports a case of bow hunter's stroke in a 25-year-old male who complained of visual disturbance and syncope on rotation of the head 90 degrees or more to the left. This problem was fre-quently seen on driving a car. A cervical x-ray and MRI of the head revealed no abnormal findings such as atlantoaxial dislocation.
