超音波血流検査が有用であった特発性内頸動脈海綿静脈洞瘻の1例

Usefulness of Ultrasonography in a Case of Spontaneous Carotid-Cavernous Fistula:A Case Report

　眼球突出，眼瞼結膜の充血，複視などで発症する特発性内頸動脈海綿静脈洞瘻（CCF）の病態は海綿静脈洞部の硬膜動静脈瘻であり，内頸動脈と海綿静脈洞に直接瘻を形成するCCFとの鑑別診断には脳血管撮影を必要とする。今回われわれは超音波血流検査がCT，MRI，MRAよりも脳血管撮影の結果によく相関すること示した症例を経験したので報告する。頸部超音波血流検査では動静脈瘻に関与している頸部動脈の血流速度の上昇・末梢血管抵抗の減少がみられ，塞栓術後にはこれらの所見は改善した。超音波血流検査はCCFの診断において有用であるとともに，外来でも施行できる低侵襲検査であるので，塞栓術後の外来経過観察にも適していると考えられた。

Since spontaneous carotid-cavernous fistula (CCF) is a dural arterio-venous fistula at the cavernous si-nus, which is different from traumatic CCF and CCF associated with a ruptured aneurysm at the cavernous internal carotid artery, cerebral angiography is re-quired in order to differentiate these condition. We here report a case of spontaneous CCF, in which a re-sult of ultrasonographic evaluation of cervical arteries well corresponded with that of cerebral angiography.
