同側性模倣性連合運動をきたしたビタミンB1欠乏性多発ニューロパチーの1例

Vitamin B1 Deficiency Polyneuropathy Presenting Homolateral Imitative Synkinesia

　症例は，56歳女性。1995年4月に四肢脱力，しびれにて発症したビタミンB₁欠乏性多発ニューロパチーの1例である。1997年7月8日，理学療法目的にて本院転院。四肢遠位側優位に筋萎縮，筋力低下があり，表在覚，深部覚とも障害があった。また両側に同側性模倣性連合運動（足→手）を認めた。入院時検査では，血液一般に異常なし。血清ビタミンB₁値は正常。針筋電図で脱神経電位，末梢神経伝導検査で複合筋活動電位の振幅が著明に低下していた。頸胸髄，脳MRIに異常なし。本例は，末梢神経または後索レベルの障害により，内側毛帯上行系を介した随意運動への抑制が上分行われず連合運動が生じたと推測した。また本例のように末梢神経レベルによる同側性模倣性連合運動は稀であり，貴重な症例であると思われた。

We report a 56-year-old woman with vitamin B₁ polyneuropathy, showing bilateral homolateral imita-tive synkinesia . Needle electromyogram revealed neurogenic changes, and the amplitude of muscle ac-tion potential was low. Sural nerve biopsy showed a marked loss of myelinated fibers, and severe axonal degeneration was diagnosed. Spinal and brain MRI re-vealed no abnormalities. In the literature, these synki-nesias were observed in patients with parietal, tha-lamic, and basal ganglia lesions and with chorea.
