再発時に心筋炎を併発した再発性Guillain-Barré症候群の1例

A Case of Recurrent Guillain-Barré Syndrome with Myocarditis

　Guillain-Barré症候群（GBS）から回復後2年間の無症候期間をおいて再発し，その際に心筋炎を合併した症例を報告する。症例は35歳男性。感冒様症状に引き続き四肢の筋力低下，腱反射低下を呈しGBSと診断され，血漿交換で完全に回復した。2年後に再び感冒様症状，心不全症状，四肢筋力低下をきたしGBSの再発と診断した。入院後4日目から筋力低下は自然に改善傾向を示したため，心不全の回復を待って血漿交換を施行し，約3カ月で完全回復した。発症1カ月の時点で施行した心筋生検では心筋炎の回復期の所見であった。心筋炎を伴ったGBSは稀で，再発性GBSに心筋炎を合併した例は本報告が初めてである。文献上心筋炎を伴ったGBSは死亡率が高く，厳密な管理が必要と考えられるが，本例は良好な経過をとった。

We report a 35-year-old man who recovered from an initial episode of Guillain-Barré syndrome (GBS) and had acute relapse after two years of asymptomatic interval. He had an acute muscle weakness with are-flexia in his extremities following an upper respiratory tract infection in 1993. He was treated with plasma ex-change and recovered completely within two months. Two years later he had a relapse of muscle weakness in the same distribution as the initial episode following the symptoms and signs of congestive heart failure.
