脳表ヘモジデリン沈着症の1例—神経耳科学的検査，誘発電位による検討

A Case of Superficial Siderosis of the Central Nervous System : Findings of the Neuro-Otological Tests and Evoked Potentials

　特発性脳表ヘモジデリン沈着症（SS）の1例を報告した。症例は57歳，女性。緩徐進行性の難聴，歩行障害，排尿障害を主訴とし，神経学的には感音性難聴，小脳性運動失調，錐体路徴候，構語障害，神経因性膀胱を認めた。髄液には異常は認めなかったが，MRIのT1, T2，プロトン強調画像にて脳・脊髄を縁どるような表層の低信号域を認めた。神経耳科学的検査では後迷路性障害を認めた。各種誘発電位検査にて聴性脳幹反応は誘発不良，運動誘発電位では特に下肢の中枢運動伝導時間の著明な延長が認められた。本症において神経耳科学的検査，運動誘発電位を検討した報告はなく，それぞれSSの病態の評価に有用と考えられるので報告する。

We report a 57-year-old woman with superficial siderosis of the central nervous system. On physical examination, sensorineural hearing impairment, cerebellar ataxia, pyramidal tract signs, dysarthria, and neurogenic bladder were seen. Brain and spinal cord MRI (T2 and proton weighted images) revealed cerebellar atrophy and marginal hypointensity of the Sylvian fissure and brain stem, cerebellum and spinal cord. We diagnosed this patient as superficial siderosis because of the clinical course and specific findings on MRI.
