　急激な経過をたどった頸部硬膜外膿瘍の1例を経験したので報告する。症例は57歳男性で，食道癌の治療のため当院に入院していた。外泊中に除雪をしていて急に腰部痛が出現し，帰院した翌日背部痛が出現，更に翌日頸部痛と高熱が出現し，傾眠的になったため，髄膜炎の疑いで当科を紹介され腰椎穿刺が施行された。その2時間後，呼吸抑制が起こり昏睡状態となった。人工呼吸器による呼吸管理の後意識や運動機能は回復したが，3日後には四肢麻痺となった。当科に転科後，頸部造影CTにて硬膜外膿瘍が疑われ，緊急で椎弓切除による外減圧およびドレナージ術を施行した。術後，解熱し呼吸機能，運動機能は徐々に改善している。病理組織学的診断は膿瘍であったが，その原因は不明であった。

Epidural abscess is very uncommon in the cer-vical region, and it is difficult to diagnose because of the diversity of its clinical course. The resulting neurological deficits remain unacceptably severe because of delays in diagnosis and treatment. We present a rare case of acute cervical epidural abs-cess.

A 57-year-old man was admitted to our hospital because of esophageal carcinoma. During an over-night stay outside the hospital, he experienced sud-den lumbago while removing snow at his home. The next day he developed back pain and high fever. He was drowsy but complained of neck pain.
